New Clients

Please fill out the new client form below to provide us with your information along with your pet’s information. This will allow us to ensure we’re able to meet your needs and the needs of your pet prior to scheduling an appointment.

Note we will also need any records from your previous veterinarian prior to scheduling an appointment time.

Prior to your first appointment, please request your pet’s medical records from your previous veterinarian. This may be done by placing a call or email to your veterinarian and asking them to send complete records, including any diagnostics (such as blood work, biopsies, etc.) to [email protected]. This request must come from you as these records are confidential and require your consent to be released.

An appointment will not be scheduled until these records have been received as they contain relevant medical history that will be important for the current care of your pet.

*If you submit a new client request but we do not receive records from your previous veterinarian, you will be placed on a waitlist until records have been received. Only once records have been received will you be able to schedule an appointment.*
